In the bustling metropolis of New York City, where the skyline is ever-changing and the streets below brim with ceaseless activity, maintaining the integrity and safety of the city's vast array of buildings is a monumental task. Among the myriad of documents and processes designed to uphold this integrity, the NYC Buildings OP128 form plays a crucial yet often underappreciated role. This form, specifically designed for reporting Ordinary Plumbing Work, serves as a critical tool for Licensed Master Plumbers to record and report plumbing work that does not necessitate a permit. Requiring detailed information on the applicant, location, and nature of the plumbing tasks undertaken, the OP128 form outlines a variety of plumbing work scenarios, from the removal or relocation of plumbing systems and fixtures to the installation or repair of elements such as gas boilers, water heaters, and backflow preventers. Detailed in its scope, the form ensures compliance with Administrative Code 28-105.4.4, emphasizing work that impacts the city’s water and gas supply lines. Moreover, its directive for plumbers to submit a monthly report underscores the city's commitment to regular oversight and the prevention of potential hazards. The stipulation that reports must be filed with a fee and within a specific timeframe, coupled with the penalties for non-compliance, illustrates the balance between regulatory oversight and the autonomy granted to plumbing professionals. In essence, the OP128 form encapsulates the intricate dance between maintaining public safety and facilitating the seamless operation of the city's basic services.

Common mistakes

Filling out the NYC Buildings OP128 form, an Ordinary Plumbing Work Report, is a process that requires attention to detail. However, applicants often make mistakes, some of which are more common than others. Understanding these errors can help ensure the form is completed correctly, supporting a smoother approval process.

The first common mistake is not double-checking the filing status. The form requires you to indicate whether it's an initial filing or a correction. Failing to specify or incorrectly marking this section can lead to processing delays. This detail is crucial for the Department of Buildings (DOB) to understand the report's context and process it efficiently.

Another error involves the applicant information section. It's necessary for all applications to provide comprehensive contact information, including the business name, address, and various telephone numbers. Overlooking any of these fields or entering incorrect information can hamper communication between the applicant and the DOB, potentially delaying the project.

  1. Not properly affixing the BIS license number label is a frequent oversight. The DOB requires this for identification purposes, and forgetting to affix the label, or placing it incorrectly, can invalidate the submission.
  2. Incorrectly listing location information, including the borough number, block, lot, and BIN #, compromises the report's accuracy. Ensuring this information is precise is critical for the DOB's records and any related work's legitimacy.
  3. Overlooking to check the applicable statements under the "Applicant Statements and Signature" section can lead to misunderstandings about the applicant's qualifications and the work's compliance with the building code. Failing to review and sign this section properly invalidates the report.
  4. Submitting the form without reviewing the instructions usually results in errors. The instructions contain crucial information about the form's purpose, submission deadlines, and requirements for ordinary plumbing work that does not require a permit.
  5. Using outdated information or failing to update the form with the latest departmental requirements can also be problematic. Regulations and submission procedures can change, so it's essential to use the most current form and follow the latest guidelines.

Addressing these common mistakes can significantly improve the accuracy and acceptance rate of the OP128 form submissions. It's recommended to review the form meticulously before submission, double-check all entered information, and ensure compliance with all DOB requirements. Properly filled forms expedite the approval process, allowing plumbing projects to proceed without unnecessary delays.

Documents used along the form

When working with the NYC Buildings OP128 form, an Ordinary Plumbing Work Report, it's common to encounter a number of other forms and documents that aid in the seamless processing and compliance of plumbing work within New York City. These documents not only help professionals navigate legal requirements but also ensure that all work is up to standard, safeguarding the building's integrity and the safety of its occupants.

  • DOB NOW: Build Application - This is an online platform for submitting construction documents and applications. It is used for a wide array of job filings, including those that cannot be categorized as "ordinary" and therefore, require additional review and permits.
  • Limited Alteration Application (LAA) - Required for minor alterations that do not change the use, egress, or occupancy of a building. This is commonly used for backflow prevention devices that need to be filed separately from ordinary work.
  • Permit for Plumbing Work - Although the OP128 covers work that doesn’t typically require a permit, any deviations or broader plumbing projects necessitate a Plumbing Work Permit to commence legally.
  • Insurance Certificates - Proof of the contractor’s general liability, workers' compensation, and disability insurance must be on file with the Department of Buildings to protect against potential liabilities.
  • Asbestos Investigation Report - Required for buildings constructed before a certain year to determine if asbestos abatement is necessary before plumbing work can proceed.
  • Building Information System (BIS) Property Profile Overview - Offers comprehensive data on a building's legal use, occupancy, permits, complaints, and violations. This is crucial for ensuring compliance with current codes and regulations.
  • Plumbing Inspection Request - After work completion, a request for inspection must be submitted to ensure the installation or repairs adhere to the NYC Plumbing Code. This is essential for the certification of the work performed.

Each document serves a specific purpose, facilitating adherence to city regulations and enhancing the efficacy of plumbing work within New York City. From initial applications to subsequent inspections, these documents work in conjunction with the OP128 form to ensure a comprehensive approach to building safety and regulatory compliance.

Similar forms

One document similar to the NYC Buildings OP128 form is the Permit Application for Plumbing Work. Much like the OP128 form, the plumbing permit application requires detailed information about the work's scope, the property location, and the licensed practitioner undertaking the work. Both documents are essential for regulatory compliance, ensuring that plumbing work complies with local building codes and standards. However, unlike the OP128, which reports ordinary plumbing work not requiring a permit, the permit application is a prerequisite for starting substantial plumbing installations or modifications.

The Alteration Type 2 (Alt2) Application also shares similarities with the OP128 form. The Alt2 application is used for renovations or alterations in a building that don't change its use, egress, or occupancy but may involve multiple types of work, including plumbing. Like the OP128, this form requires detailed applicant and property information and descriptions of the intended work. Both forms ensure that the proposed alterations meet the city's building and safety standards, albeit the OP128 is specifically for plumbing work not requiring permits.

The Certificate of Correction shares a purposeful relationship with the OP128 form, though it serves a distinct regulatory function. This document is used to certify the correction of cited violations, similar to how the OP128 might report corrections of non-compliant plumbing work. Both forms are pivotal in maintaining buildings within the permissible standards of New York City's Building Code, with the Certificate of Correction being a follow-up document ensuring compliance after violations have been identified.

Another related document is the Limited Alteration Application (LAA), particularly for plumbing and sprinkler work that doesn't necessitate a full Alteration Type 1 or 2 filing. The LAA and OP128 forms facilitate minor modifications or repairs, streamlining the approval process for specific work types. Both require detailed project and licensee information, yet the LAA is broader, encompassing a range of minor alterations, including but not limited to plumbing.

The Construction Superintendent Initiation/Cancellation Form, while not exclusively tied to plumbing, serves a similar logistical role by providing the Department of Buildings (DOB) with critical project oversight information. This form and the OP128 both contribute to the DOB's regulatory functions, with the former ensuring qualified supervision over construction sites and the latter reporting plumbing work that falls under ordinary maintenance and repair.

Lastly, the Asbestos Project Notification Form (ACP7) provides an interesting parallel. Before any renovation or demolition, including plumbing work reported on an OP128 form, the presence of asbestos must be assessed and managed. The ACP7 is required if asbestos is involved, to ensure proper handling and abatement procedures are followed, highlighting the interconnectedness of building safety, public health, and even routine plumbing works within the regulatory landscape.

Dos and Don'ts

Filling out the NYC Buildings OP128 form, an Ordinary Plumbing Work Report, is a crucial task for authorized plumbing work in New York City. • Do ensure the application is typewritten to maintain clarity and professionalism.

  • Do affix the BIS (Building Information System) license number label in the designated area on the form, as this is crucial for the application’s processing.
  • Do provide all the required applicant and location information comprehensively, including last name, first name, middle initial, and the specific details about the job location and scope of work.

  • Don’t forget to check if certain applicant statements apply, such as the operational condition of the chimney when replacing gas equipment, and sign the statement section to attest to the accuracy of the information provided.
  • Don’t overlook the deadline to submit the completed form and required attachments to the DOB by the 15th of the month following the reporting period to avoid penalties or violations.
  • Don’t fail to include a self-addressed, stamped envelope with your report submission if mailing your documents, as this facilitates the return of processed documents.
  • Don’t submit without double-checking that your license and insurance are active at the time of filing, as this is a prerequisite for the form's acceptance.

Following these guidelines diligently will help in the smooth processing of your plumbing work report, ensuring compliance and avoiding potential setbacks or penalties from the NYC DOB.

Misconceptions

When it comes to the NYC Buildings OP128 form, used for reporting ordinary plumbing work, there are several misconceptions that can lead to confusion for both professionals and property owners. Here are ten common misunderstandings:

  • The OP128 form is optional. Many people believe filing the OP128 form is a choice. In reality, Licensed Master Plumbers must use this form to report specific types of plumbing work to the Department of Buildings (DOB), as outlined in the NYC Building Code section 28-105.4.4.
  • Any plumbing work can be reported on the OP128. This form is intended only for ordinary plumbing work as per the specified code. This includes tasks such as replacing a gas boiler or water heater under certain BTUs, relocating plumbing fixtures within 10 feet, and repairing non-gas, non-fire suppression piping not longer than 10 feet, among others.
  • A permit is always required for work reported on the OP128. A key feature of the OP128 form is that it's used for plumbing work that does not require a permit, contrary to this common misconception.
  • There's no deadline for submission. The form must be submitted by the 15th of the month following the month in which the work was performed. Late submissions can result in violations or disciplinary actions.
  • Any plumber can file the OP128 form. Only Licensed Master Plumbers can file this form. This certification ensures the individual has met specific experience requirements and understands the NYC Building Code related to plumbing.
  • Digital submissions are acceptable. As of the last known update, the form must be typewritten and submitted with a physical copy, including a license number sticker, to the Department's LAA unit. Digital submissions are not outlined as acceptable in the instructions.
  • The form is single-sided. In fact, the application must be double-sided according to the instructions. Attention to this detail is crucial for proper filing.
  • All sections of the form are compulsory. While most of the form needs to be completed, certain fields or sections might not apply to every submission. For example, the correction reference number is only required if the filing is a correction.
  • Work beyond 20 activities can't be reported. The form limits the report to a maximum of 20 activities per Ordinary Plumbing Work Report. If more work needs to be reported, additional forms should be used.
  • Failure to file has no consequences. This misunderstanding could be costly. Failure to submit the form or late filing can lead to violations and disciplinary actions against the plumber.

Understanding these misconceptions about the NYC Buildings OP128 form can help Licensed Master Plumbers and property owners navigate the reporting process more effectively, ensuring compliance with the Department of Buildings' requirements.
